Social Determinants of Health
CEU Description for Website: The Centers for Disease Control & Prevention (CDC) defines Social Determinants of Health (SDOH) as “conditions in the places where people live, learn, work and play” which “affect a wide range of health risks and outcomes.” It is increasingly recognized that these conditions are directly related to both adherence and health outcomes and that they should thus be an integral part of treatment planning. Please join Peer Services as we explore the five major SDOH, how to asses for them, how to incorporate them into the treatment plan, and how Peers can assist individuals in meeting these needs early in the treatment process and thus improve individual outcomes. We are excited to have guest speakers from DSAMH who will talk about some of the initiatives here in Delaware. They will be available to answer your questions as well.
