Precision Dance Collective

Shelly Hutchinson is a choreographer, educator, dancer,
actress and producer in New York City. She has been a
Contemporary and Musical Theater choreographer for “So
You Think You Can Dance” Ukraine for the last 5 seasons,
also on the creative teams and performed in operas such as
Candide, Samson and Delilah, and Aida as associate
choreographer and assistant director. Choreographer for
adapted for stage version of Disney’s High School Musical.
TV appearances include MTV, CBS Morning Show, ABC,
Saturday Night Live, Law & Order, Lipstick Jungle, Ugly
Betty, Kings, HBO Miniseries “Mildred Pierce”, The Today
Show, David Letterman, The Learning Channel, dancer for
Wyclef Jean and in Lou Bega’s “Mambo #5”. Performing
credits include Radio City Music Hall, Warner Brothers Films,
Disney, Off- Broadway and Regional Theatre, Broadway
Bares, International Arts Festival in Italy, Care Wave Japan,
National and European Tours including “Fame, the Musical”,
Bye Bye Birdie, Irene, and West Side Story, assistant
choreographer for “Falco”, a multi- media show in Vienna,
Austria based on the late 80’s pop star.

Her work has been seen at City Center, The Apollo,
Symphony Space, Merce Cunningham, and New York Live
Arts. With 25 years of deep and diverse teaching experience
with all levels and ages, she has taught at various schools
and conventions in the US, Germany, Italy, Austria, Prague,
Argentina, Japan, Costa Rica, Cirque Du Soleil, in Las
Vegas, Rutgers University Dance Team and University of
MD. Audition choreographer for Disney, Royal Caribbean
Cruise Lines, New York Fashion Week, Choreographer for
“Nix” and “Occasion for Sin” and Nominated for “Outstanding
Choreography” as part of Planet Connections Theater
Festivity.

Currently on faculty at Pace University-Sand College of
Performing Arts with the Musical Theater and Commercial
Dance Department, Manhattanville College, Broadway
Dance Center. and Joffrey Ballet School in NYC. Currently in
certificate training for trauma informed teaching practices,
her production company “Royal City K Productions” focuses
on producing live events and performances for local an
international Charities.

By registering, participants grant permission for Precision Dance Collective to capture and use photo/video content fully competent to enter into this waiver.

LIABILITY: 18+

I acknowledge that participation in classes, workshops, rehearsals, performances, and related activities offered by Precision Dance Collective LLC involves physical activity and inherent risks, including but not limited to bodily injury, illness, permanent disability, property damage, or other losses.

In consideration for being permitted to participate, I voluntarily assume all risks associated with participation in any Precision Dance Collective LLC activity.

To the fullest extent permitted by law, I hereby release, waive, and discharge Precision Dance Collective LLC, including its owners, directors, faculty, staff, contractors, agents, representatives, successors, and assigns (collectively, the “Released Parties”), from any and all claims, liabilities, demands, causes of action, damages, losses, or expenses of any kind, whether known or unknown, arising out of or related to my participation in any program, activity, or event, including those that may arise in the future.

This Release applies to all claims, including but not limited to those arising from negligence, to the fullest extent permitted by the laws of the State of New York.

I understand and agree that:

• I am physically able to participate and have not been advised otherwise by a medical professional.

• I am responsible for monitoring my own physical condition and stopping activity if I feel unwell or unsafe.

• Precision Dance Collective LLC reserves the right to remove any participant whose conduct poses a safety risk to themselves or others.

This Waiver and Release constitutes the entire agreement between the parties and shall be governed by the laws of the State of New York. If any portion of this agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect.

By signing below, I acknowledge that I have read this Waiver and Release in its entirety, understand its terms, and agree to it freely and voluntarily. I understand that I am giving up substantial legal rights, including the right to sue.

I certify that I am 18 years of age or older and legally competent to enter into this agreement.

LIABILITY: MINOR

I, as the parent or legal guardian of the above-named participant, acknowledge that I have read and understand this Waiver and Release of Liability. I consent to the minor’s participation in Precision Dance Collective LLC activities and agree, on behalf of myself and the minor, to be bound by all terms and conditions stated herein.

I further agree to release and hold harmless Precision Dance Collective LLC and the Released Parties from any and all claims arising out of or related to the minor’s participation, to the fullest extent permitted by law.
