* This form is used to enter the entries for the 5/1 - 5/3/2026 National Middle School Tournament of Champions (MS TOC) competition. Please read the following instructions carefully.

QD Tournament Participation Requirement
- Students who are in our Varsity Team
- Tournament Participation is subject to Coach & Admin Approval

Who is eligible to participate?

- Students Grade: 6th-8th
- Students on the Varsity Team Only/ Coach Approved Students
- All entries will be reviewed for approval by our Tournament Lead coaches

How to fill this out?

- Please fill out one form for each participating student.
- Select the event the student will be competing in.

Deadlines:
- Register by 4/24/2026 by 11:59pm. Payment by 4/26/2026 by 11:59pm.
- Any late sign ups will not be accepted.
- Any changes and drops after the deadline will incur a minimum $100 fee. Fees vary by tournament.

Private Coaching Virtual Sessions:

Starting rate is $140/hr and can be up to $150/hr depending which coach is selected to run the private coaching session. Minimum 1.5 hours required.
