* This form is used to enter the entries for the 5/15 - 5/17/2026 CHSSA's Middle School State Championships competition. Please read the following instructions carefully.

QD Tournament Participation Requirement
- Students who are in our Varsity Team
- Students who reside in California
- Tournament Participation is subject to Coach & Admin Approval

Who is eligible to participate?
- Students Grade: 6th-8th
- Students on the Varsity Team Only
- All entries will be reviewed for approval by our Tournament Lead coaches

How to fill this out?
- Please fill out one form for each participating student.
- Select the event the student will be competing in.

Deadlines:

- Register by 4/28/2026 by 11:59pm. Payment by 5/1/2026 by 11:59pm.
- Any late sign ups will not be accepted.
- Any changes and drops after the deadline will incur a minimum $100 fee. Fees vary by tournament.

Tournament Fees
Tournament Service Fee: $50

Entry Fee Per Event for Speech: $50
Judging Fee for each SPEECH event: $125 (judging fee will be waived if parents can be judges for all required rounds on both days)

Entry Fee Per Event for Congress Debate: $50
Judging Fee for each CONGRESS event: $375 (judging fee will be waived if parents can be judges for all required rounds for 3 days)

**Drop deadline is 5/1/2026 at 11:59PM. No refunds will be issued after the drop deadline.**
